We please request that you give Advanced Massage Therapy 24 hours notice if you must cancel your appointment. There is a late cancelation fee, or a last minute no show fee of 50% of the services requested. We are a busy clinic, and treatment spots are in demand. Are you covered? Advanced Massage Therapy can direct bill at no additional charge to the following private insurance companies. If Massage Therapy is covered in your plan, please view the list below to find the required forms for your Private Insurance.
Advanced Massage Therapy is Happy to Direct Bill for its clients. However it is the responsibility of the client to know exactly what their insurance covers, and to pay all extra fees EX. Deductables, and also make sure you have a Doctors Note ( and know when it expires ) Please let us know if you will be using insurance, and come prepared to your appointment with ALL INFORMATION regarding your plan. You will be asked to fill out an ADVANCED MASSAGE INSURANCE POLICY, so we can better help set you up for successful Online Billing in our Clinics. All REJECTED claims will have to be paid in full, as it is the Policy owners responsibilty to be aware of its usage, and its expirey or termination dates.
